Vial of L.I.F.E.

A form, which you fill out with Information important to your health care, such as your doctors name, allergies, medications, pertinent medical history, living will, DNR paperwork, emergency contact information etc., is placed in a magnetic plastic pouch.

This pouch is placed on the outside of your refrigerator so that in a medical emergency our emergency medical personnel can have quick access to this information even in a situation where you may not be able to provide it.

We have provided the Vial of L.I.F.E. Forms in both English and Spanish here in Adobe Acrobat format.
